Community Health Plan of Washington offers two programs for Medicare members to complete an annual health assessment. The goal of these programs is to help members learn more about their health and offer programs, services and information to best meet their needs.
One of these programs is an annual health appraisal. This is a survey that is first offered over the phone. If we can’t reach a member over the phone a survey is sent in the mail. We are required to offer members this survey each year and do our best to make taking the survey easy. After finishing the survey we send information to each member based on their answers.
Another program we offer members is an in-home health assessment. In this program a nurse will go to a member’s home to discuss health history and any health concerns or conditions. The nurse will also make referrals for members to programs or other health care providers. We partner with Matrix Medical Network to offer this program and they reach out to eligible members to schedule visits.
Members can choose to participate in these programs when they are contacted. Members are not required to do so. These programs do not replace seeing your doctor regularly for well care or when you have health concerns. It is important to have a good relationship with your doctor and share information about your health history and anything you may learn through these additional programs.
